The original plan was for Scott Derrickson to take the helm of the Doctor Strange sequel, though "creative differences" with Marvel Studios led to him departing the project. The belief is that Marvel Studios' focus on the Multiverse played a role in the filmmaker leaving the MCU, and it's previously been reported that Derrickson was looking to focus on Nightmare.

The 2016 movie originally featured a flashback sequence to the night Stephen Strange's sister drowned in an icy lake, an event that was referenced in Doctor Strange in the Multiverse of Madness.

However, concept art shared by Oliver Carroll has now revealed a scrapped confrontation with Nightmare! It's unclear whether this is from when Derrickson was still attached to the project or an early Sam Raimi idea, but it looks like Strange would have been forced to return to the fateful night his sister died. 

We've got no idea who those beings on the shore watching on are, but can safely say that is Nightmare due to the demonic villain being sat atop a horse.

Perhaps the Scarlet Witch originally intended to set Nightmare after the former Sorcerer Supreme before Marvel Studios decided the villain could be better used in a future movie? Either way, it did look like there was a time when Strange's dreams were an important part of this movie, though they ultimately became a way for him to see into the Multiverse.

@ScaryTerry - Ever since Dr Strange was a thing, I wanted nothing more than Nightmare as a villain. The audience doesn't know or truly understand Strange's past and it could have been a great moment to add impactful emotion and to not only the character but the audience as well through Nightmare. Them brushing over his sister in barely a minute without any flashbacks or anything pissed me off. His past is pretty darn dark not just his sister, his brother, his parents and you can understand why he was initially an @sshole. In the MCU we know he is one, they just failed to show us why. Audiences would love those moments. It would have made the movie stand out in its genre when you delve much deeper into a character. Now it's just another mediocre/decent at best CBM. As much as it pains me to say this, as a fan of the character. Maybe, just maybe, they have signed Benedict to do MANY solo Strange movies, like 5+ movies at least and they decided they have enough solo films lined up to waste MOM with a major focus on SW and they will focus on Strange only in his next outings. This is my copium at least.

@Typhoon20 - total missed opportunity. At least set it up. Especially with the sister story, would have hit much harder if we witnessed the tragedy and flashed Nightmare at the last second as a setup

@Typhoon20 - Speaking of missed opportunities. That first episode of She-Hulk was so rushed. They glossed over what could have been a complex character study.

I really think they wanted Scarlet Witch as the villain of the movie, while Derrickson wanted Nightmare. And judging by the looks of it, Nightmare could've led to some cool stuff too.

Nightmare could return though, maybe in a What If story (written by Derrickson?)

I like this movie a lot. I liked it more the second time....I understand most of the gripes but I just can't seem to give a crap about the flaws with DSMOM. I remember all my favorite comics as flawed masterpieces, sometimes the artwork would redeem the story of vice versa

@Typhoon20 - I'm a very critical person mostly. The MCU is a childhood dream come true and I don't want to spoil it for myself by treating it like it was intended to be epoch defining art. And at it's worst it's worlds better than those transformers flicks

@DoubleD - They are unhulking the HULK. I suppose it's an updated version of the brown uniform with the black shoes. I wish we would've gotten more time with the wild man. I always liked Professor HULK in the 90s, but this doesn't feel earned.

@VictorAlonzo - She-Hulk episode one was AWESOME also explained how Hulk has Multiple Personalities. Hulk not a version but has multi-Personalities disorder and why we get different Hulk in all the movies.

Also, why people like Professor X can't read his mind or control him.

1) Incredible Hulk personality
2) Avenger's personality
3) Age of Ultron personality
4) Thor Ragnarok personality
5) Endgame personality

Now She-Hulk all the same Hulk. I think the Hulk will get angry and get tired of being controlled by Banner :)

The multiple HULK personality is a lazy way of giving writers free reign on prostituting his character to fit whatever need. HULK has been: Angry, Angrier, Angriest, Joe Fixit (who I think would be fun with tenor aguments of Joe Pesci), Professor HULK, The Maestro, and Banner.

The development on Planet HULK was well earned due to the excellent situational writing of Greg Pak, and the Age of Ultron was mind manipulation.

I liked the idea of HULK utilizing subliminal genius physics to never fully injure innocent people. I forgot where I first read it, but that was cool as well. Other than these examples, I call mulligans on this new veersion of his split personality disorder.

@VictorAlonzo - Hulk Personalities is what makes the Hulk the Hulk. It was not the gamma bomb radiation that screwed up Banner’s mind. Before Bruce became the Hulk, he already had a lot going on in his life. Past childhood trauma on account of an abusive father had led a young Bruce Banner to develop dissociative identity disorder. His mental condition went undiagnosed for years and was later repressed by an adult Banner.

After the gamma bomb radiation turned him into the Jade Giant, Banner’s mental condition only worsened. He developed multiple Hulk personas – Joe Fixit, Savage Hulk, Merged Hulk/The Professor, and Devil Hulk are only the tip of the iceberg.

@RongDesign - I remember Derrickson mentioning at one point that it would be interesting to explore Strange's guilt in his part of knowing Stark would need to make the sacrifice for them to win.

Shame we didn't get any of that in MOM. missed opportunity

That’s precisely what it should have been.

They already did the Wanda grief story in Wandavision and ended with her reading the Darkhold.

I figured they would have her corrupted by Nightnare and he was the cause for her doing so many horrible things.

Then She redeems herself by breaking from Nightmare’s control and working together with Strange to defeat him.

There is her redemption arc. However, as they wrote it, Wanda cannot be redeemed.

Nightmare should have been the main villain,you could have still involved Scarlet Witch looking for a children but Nightmare could have been messing with her head.

I like the introduction of new villains. Could have been cool.

Also, so glad they didn’t kill off 616 Mordo like originally intended. He NEEDS to come back as main villain of DS3. That arc needs payoff. The bill comes due.

I like MOM, but it did feel like something that was heavily edited down and streamlined. Plus everyone had all these super high expectations which didn't help. I tend to prefer self-contained, personal stories, but this felt a little like a let-down after all the hype.

Not sure if adding this would have changed anything. So Scarlet Witch would be controlled by both the Darkhold and Nightmare?

Since Multiverse is one of my least favourite MCU movies I can't say I wouldn't have prefered to see Derrickson doing the sequel he wanted. It's highly unlikely I would've liked it less.
