The film starts off with Mephisto narrating how evil souls escape Hell from time to time and how he must convince someone to become the Ghost Rider. After many different incarnations, Mephisto speaks of how Johnny Blaze stole his curse, but continues to send souls to Hell. Mephisto then says that one day Blaze will give up the mantle of the Ghost Rider, but it is not anytime soon. We then enter a chase with a criminal and a bounty hunter. The bounty hunter brings the criminal to jail, where a cell mate recognizes him (the bounty hunter) as Johnny Blaze. Johnny says that he gave up being a daredevil when he found his true calling as a bounty hunter. Later that night, Blaze stops at a bar where a mugging takes place. Blaze transforms into Ghost Rider and gives the mugger the Penace Stare. The mugger's soul is then taken to Hell as his body is left behind. Johnny returns home and looks at a picture of his lost love, Roxanne.
Elsewhere, a breaking and entering takes place at a local home. One of the residents, Daniel Ketch, fights them off but is wounded and is forced to watch them cripple and rape his sister. The cops arrive before the burgulars have a chance to kill them. Weeks go by and two of the four criminals are caught, with one skipping trial, forcing Blaze to catch him. Danny is upset as the cops don't have all the criminals. Blaze says that Ketch should not look for revenge. Ketch, dissapointed, goes to ride on his motorcycle. However, he becomes possessed by it and transforms into another Ghost Rider. Mephisto comes to Earth and tells Blaze that the demon Zarathos had escaped Hell and hid in Danny's motorcycle. Mephisto says that Blaze must bring Zarathos back to Hell before he can do serious damage. Johnny finds Danny and the two fight, showing that they are matched opponents. Johnny prepares to give Danny the Penace Stare, but can't since it is still Danny, a person he has grown to consider a great friend. Danny, overpowered by Zarathos, defeats Johnny and rides into the night. When Danny takes control again, he plans to use his powers to find the criminals that raped and crippled his sister. Johnny goes looking for Danny and discovers that he found the criminals and brutally killed them. Convinced that Zarathos has taken full control, Johnny plans to defeat Danny and to stop all the killing.
Danny stays at a motel that night and questions his actions. During his dreams, he is told by a vision of his sister that he has disappointed her. Also, Zarathos becomes a vision as well and fights Danny in his dream. Danny loses and Zarathos takes full control. Zarathos burns down the motel and goes on a rampage, summoning the dead from graves and brining evils souls to Earth from Hell. Johnny defeats most of the souls in a large battle before confronting Danny/Zarathos. They have a long fight, but Zarathos overpowers Johnny. As a last resort, Johnny uses the Penace Stare, which kills Danny. Zarathos, wounded by not having Danny anymore, is defeated by Johnny and sent back to Hell. Mephisto returns and tells Johnny that he will ressurect Danny if Johnny gives him back the Ghost Rider powers. Johnny, wanting to do something good and just, gives in and Danny is brought back while Johnny's power is taken. While Johnny returns home, Mephisto asks Danny if he would like the power of the Ghost Rider and to do his (Mephisto's) bidding. In return, he will give Danny's sister back her legs. Danny agrees and becomes Ghost Rider. Johnny returns home and reunites with Roxanne, while Mephisto narrates and sinisterly says that anything can be solved with a deal.
After the credits, we see the demon Vengeance, one of the souls that escaped Hell, on the loose.
