In the end of the X-Men: The Last Stand, the after credit scene, his mind takes control of a comatose body. Earlier in that same movie he was giving a lecture to students in which he states that some mutants have strong psychic abilities that they can transport their conciousness to a body that is brain dead (or something of the kind).

On how/why does he still looks the same? It could be some mental ability or something that he just allows people to see him the same (just speculation). There is a chance we will never know now since the events of X-Men: The Last stand never took place and he never really died to begin with.

That's no excuse for poor story telling. They have explanations for how mutants came to be, they sorted out a lot of the issues with previous movies but failed to acknowledge this one issue.

I feel as though a lot of things, including the Rogue escape scene got left in the cutting room floor

They explain the Xavier thing in the commentary of X3. The guy in the coma is Xavier's twin brother, who was born brain dead because Xavier's mental powers were so strong that there wasn't enough room in the womb for both brains.

The Xavier thing was explained in The Last Stand (and the identity of the comatose man was confirmed in the commentary on the DVD).

Re Wolverine's claws, an explanation would have been nice - but it's not difficult to infer what happened (Magneto recoated them at some point in the 8 years between The Wolverine post-credits and DOFP).

Wolverine's claws. Singer suggested that Magneto applied a new coat of metal. There is no confirmation that it was adamantium but this is plausible as Magneto stated in X2, "William Stryker, the only OTHER man I know who can manipulate adamantium." This implied that Mags can manipulate the metal as well.

Frick. Hugh Jackman is one of my favorite actors. He plays Wolverine really well and well, he's just a BA, basically. I wish I could work with him at some point in a film.
