The Boys season 3 is now available on DVD and Blu-ray, and Games Radar has debuted a new deleted scene from the season finale.
Towards the end of the episode, Billy Butcher is given a grim diagnosis after being examined by the doctor. It seems his use/abuse of V-24, aka Temp-V, has done permanent damage, and Butcher's time is running out.
In this extended version of the scene, Hughie drops in for a visit after being told he's going to be okay ("They said it will be a couple of months, a lot of migraines, but I’m mostly all good,") and Billy decides to without the truth from his teammate.

Butcher's imminent death and refusal to come clean about it is certain to be a major plot point of season 4. If you've read the comics, you'll know that things go down a very dark route, but (so far, at least) the Prime Video adaptation has been a little less nihilistic than its source material.
Season 4 plot details are a mystery for now, but we do know that new recruits will include Susan Heyward as Sister Sage, Valorie Curry as Firecracker, and Jeffrey Dean Morgan in an undisclosed role. Cameron Crovertti (Ryan) has also been upped to series regular, which is hardly surprising after the sinister final shot of season 3.
Showrunner Erik Kripke recently confirmed that The Boys will not finish up after four seasons.
